Internet Access Form

As an attendee of a conference/event/meeting held on the NAU campus, you have the option to access the Internet via an NAU network using a "wired" (Ethernet card) connection through an active data jack OR you may use your "wireless" card when staying in the residence hall. In either case, you must complete and submit this form.
Note to use your wireless card in public spaces on campus you do NOT need to use this form. Instead find the GUEST network.

Your computer's "Physical Address":
Be sure to enter either the correct number - either Ethernet "Wired" OR your "WIRELESS" physical address.

Guidelines & Prohibitions:

The Internet is a world-wide telecommunication network. There are hundreds of resources on the Internet. You will find libraries, government agencies, universities, discussion groups, software, technical information, as well as over 20 million people who are part of this world-wide network. Access to the Northern Arizona University system is a privilege available to qualified campus guests. Because of this, you have a responsibility to help maintain this resource. The system you will be accessing and the networks to which they are attached cost real dollars to maintain. Reliable operation of these systems and the responsible use of their resources is of common interest and concern for all users, operators and sponsors. In addition to ethical concerns, abuse of this system could result in criminal or civil prosecution. Your use of the network is a privilege. You are responsible for your MAC address access or modem dial-up. Access granted may only be used by the authorized user.

Examples of Misuse of the Network

  1. Using someone else's network access is not acceptable.
  2. Using your network access for activities that are not conference or education-related is unacceptable.
  3. Transmitting offensive, harassing statements is not acceptable.
  4. Transmitting inappropriate graphics is not acceptable.
  5. Transmitting sexual or ethnic slurs, or sexist jokes, is not acceptable.
  6. Log off (or power down) your computer when you are not using it to prevent unauthorized use of your computer by anyone else.

Penalty for Misuse
Violating the guidelines, prohibitions, or the Student Conduct and Ethics Rules may result in loss of your network access privileges.
