**************************************** Classroom equipment loans MUST be requested by Faculty members, NOT students. **************************************** Classroom equipment will NOT be released to students (unless they are simply your courier) without receipt of this form and verbal (direct-to-me) Faculty approval and avowal of responsibility for improper damage or loss (ergo, classroom equipment must be returned in good condition with allowances for normal wear and use). Faculty members must assume responsibility for the safe and timely return of all classroom equipment entrusted to them, and consequently, for the actions of the students they may choose to themselves entrust.
