Complete your graduation application using the following instructions. Please read these instructions carefully. Do not follow the instructions from the registrar's Web page that come with the downloaded graduation application. SBS is part of a test project and requires a different set of instructions.

Complete the application using BLACK INK OR TYPEWRITER.
- Receipt Number—Write the number from your receipt for payment of the graduation fee on this line.
- Catalog of Graduation—Write the catalog year of the degree requirements you have followed on this line. You may verify the catalog year by looking on the degree progress report.
- Contact Phone Number—Write a phone number where we can reach you or leave a message from now until 2-4 weeks after your last class is completed.
- Full Name—Write your name as you wish it to appear on your diploma. If you have changed your name, please file a change of name request at the registrar's office.
- NAU ID#—Enter your nine-digit NAU ID on this line.
- Permanent Address—Enter the address where your diploma and final official transcripts should be sent.
- Degree Type—Enter the type of degree you are earning. A Bachelor of Arts (BA) requires four semesters of a language other than English; Bachelor of Science (BS) does not. A Bachelor of Social Work (BSW) is a professional designation. We also offer a Bachelor of Arts in Liberal Studies (BA) and a Bachelor of Applied Science (BAS).
- Term of Graduation—Enter the semester in which your last class will be completed. Degrees are awarded for spring, summer, fall, and winter. Commencement ceremonies are held at the end of spring and fall semesters.
- Dual Degree—Circle either yes or no. A dual degree means that you are earning two types of degrees; a BA in English and a BS in Criminal Justice, for example. You will need to file two applications, one for each degree. A dual major means that you are earning one type of degree with two majors; for example, a BS in Criminal Justice and a BS in History. You will need only one application for a dual major.
- Major/Plan—Write the name of your major, and then enter the major code. The codes are available on the Degree Progress site. If you are completing a dual degree or dual major, enter the name and code for the each degree or major.
- Adviser—Enter the name of your faculty adviser. If you have not chosen one before now, select a permanent faculty member in your major area and ask if he or she would be willing to sign your graduation papers. If the faculty member agrees, enter his or her name and phone number. Enter the name and phone number of a faculty adviser for your second degree or major as well. If you are still at a loss for an adviser, see the department chair in your major field.
- Minor—Enter the name of your minor and the name and phone number for a permanent faculty member in your minor field. If you have not selected a minor advisor, ask the department chair in your minor field. (Pam Stinson-Tattersall and Matt Pavich can sign for Criminal Justice, Psychology, and Sociology Minors.)
- Emphasis—If you are pursuing a degree that has a declared emphasis, enter the name of the emphasis on this line. You may also cross out “emphasis” and write in your second minor if applicable.
- Certificate—Enter the name of the certificate if you are pursuing one and answer the question directly below by circling yes or no. If you are earning more than one certificate, a separate application must be completed.
- Courses in the White Box—The white box is where you will list all courses that must be completed before your degree will be awarded. This means all the classes for which you are currently registered, any classes that you may still need to take, or any classes that you have taken at another college or university and have not yet transferred to NAU. You may need additional space to list classes. Attach a word-processed list of classes in the same format as the white box.
- College/Univ Name—this is the name or abbreviation of the institution where you are taking or took the class; i.e., NAU
- Subject and Catalog #—Enter the course number; i.e., CJ345W
- Unit Hours—this is the same as credit hours
- Term/Year—the semester or session you took the class
- Final Column—please see the legend or instructions to the right that specify what to put into this area. If you are still confused, do notput anything in the column.
- Requirements-Met Boxes—Leave these blank. They will be used when the graduation check is done by the Office of Academic Services and Advising or your faculty advisor.
- Total Hours—Please complete these lines following the directions and enter your current GPA. You may verify your hours completed by referring to the degree progress report.
- Student Signature—Sign and date the application and deliver as follows:
Mountain Campus Students
Drop the completed application at the Office of Academic Services and Advising in SBS or the Office of Academic Services and Advising in the School of Communication. Do this before getting signatures from your faculty advisors.
Distance Students
Submit to your advisor or local area coordinator for a signature and have him or her send the completed graduation application packet to the Office of Academic Services and Advising in SBS or the School of Communication.
Final Signatures
Mountain Campus Students:
You will be notified by NAU e-mail or phone when your application has been reviewed and is ready to be picked up.
- Obtain your final signatures in the following order: minor advisor, major advisor, department chair for your major.
- Drop signed, completed graduation application and all other attached material in the SBS Office of Academic Services and Advising for the associate dean's signature SBS WEST (bldg. 70), room 114 or in the director's office in the School of Communications (bldg. 16), room 301.
- You have filed your application. You will receive an NAU e-mail from the graduation office when your application is processed by the Office of the Registrar.
Distance Students:
Your completed, signed application packet will be sent to the Office of Academic Services and Advising in SBS or the School of Communications. Once checked, it will be sent for the appropriate final signature from the associate dean. You have filed your application.
