Are you.....

  • a person with a developmental disability?
  • a family member of a person with a developmental disability?
  • receiving home based services through the Division of Developmental Disabilities?
The goal of the EAzI project is to improve the services that you receive by creating and testing a new way to gather information about the quality of Division of Developmental Disabilities home based services. Between May 2006 and June 2007 the EAzI project will interview people with disabilities and family members about the quality of the home based services they receive through the Division. There are two ways for you to help:
  1. Become an EAzI interviewer
  2. Volunteer to be interviewed about the quality of home based services you receive
