Funding Assistive Technology through the Arizona Department of Health Services (ADHS) Children’s Rehabilitative Services (CRS)

The CRS provides a variety of medical treatments and services for individuals from birth to age 21. CRS accepts any child under the age of 21, residing in Arizona, who meets the medical criteria and whose family meets the financial criteria established by ADHS. CRS services are provided to individuals who have medically disabling, or potentially disabling conditions, as well as those who have the potential for functional improvement through medical, surgical or physical therapy treatments.

Eligibility

Any child wishing to apply for care funded by CRS must submit a complete medical history and be referred to a CRS program site for a review of medical eligibility. An initial diagnostic evaluation may be performed through CRS, if the individual is not AHCCCS eligible, or does not have other insurance coverage. One can apply at any CRS program site, or be referred by any Arizona Department of Economic Security (DES) office. Contact the closest CRS clinic for further information.

CRS will accept medically and financially eligible children who are chronically ill, or who have physically disabling conditions which have the potential for cure or significant improvement through medical, surgical or physical therapy.

DES workers onsite at the CRS clinics will determine whether you are financially eligible for CRS services, and any payment obligations based upon your family's adjusted income and other criteria.

Coverage

Medical conditions covered by CRS include:

  • congenital anomalies
  • acquired handicaps or potentially disabling orthopedic, neurological and cardiac disorders
  • connective tissue disorders
  • central nervous system degenerative disorders, such as muscular dystrophy
  • cystic fibrosis
  • sickle cell anemia, and
  • “sequelae” of trauma (abnormalities from a previous trauma). these include cerebral palsy, spina bifida and neuromuscular disorders)
  • hereditary and idiopathic disorders of the nervous system
  • chronic eye and ear disorders which could lead to blindness or deafness
  • scarring which could cause deformities, including burns
  • phenylketonuria (PKU) and other congenital metabolism disorders

Certain conditions such as infectious diseases and burns, as well as certain procedures such as organ transplants are not covered by CRS. CRS-funded medical, surgical and dental services are provided through any one of four regional contracted clinics. CRS does not cover acute, primary care health care, or emergency transport other than authorized transfers between CRS contract facilities.

Assistive Technology-related services provided by CRS include:

  • diagnostic services, including laboratory, pathology, and radiology procedures
  • rehabilitative services, including physical, occupational, speech and
  • audiological services for evaluation, re-evaluation, treatment, consultation and follow up,
  • to maintain or improve, the physical status and capabilities of the child
  • rehabilitative evaluations to assess mobility, communication device and activities of daily
  • living (ADL) needs
  • loaned augmentative communication devices
  • appliances, equipment and device modifications necessary to improve mobility or function,
  • home health care, and service coordination for families
