AT LOAN AND DEMONSTRATION PROGRAM

·        Equipment/devices are loaned for a period of 2 weeks (longer loans may be available depending on circumstances)

·        All borrowers must sign a simple loan form agreeing to be responsible for the equipment/devices while in their possession and/or to replace/repair the item if lost, damaged or stolen

·        Borrowers use all equipment/devices at their own risk

·        Individual equipment/device demonstrations or trainings can be arranged depending on needs by contacting AzTAP

·        Equipment/devices can be picked up at AzTAP or be shipped via Fed Ex to the borrower depending on their location in Arizona

·        AzTAP’s lending and demonstration program serves all areas of Arizona

·        The AzTAP AT Lab and the equipment/ devices are available to therapists throughout the community to do evaluations 

·        At this time there are no fees to borrow equipment/devices from the AzTAP AT lending and demonstration program

 

AzTAP’s AT lab is located at 2400 North Central Avenue Suite #300 Phoenix Arizona 85004.
