Referral Instructions
If you get a Referral to the Substance Abuse Program at Counseling & Testing for an Alcohol and Drug Screening:
- Come to the Counseling Center (building 26A), with a copy of your referral form within 5 working days.
- Once you arrive at the Counseling Center, you will need to pay your $50 fee and complete an online survey and an Alcohol and Drug Inventory (not online).
- Once you have completed the survey and the Alcohol and Drug Inventory, you will be given an appointment to see a Substance Abuse Program counselor. This appointment will be scheduled within one week of your starting the counseling process.
- Your first meeting will involve gathering information and more details about the incident that generated your referral. There will also be more questions about your alcohol and drug use, family history, prior incidents, present stressors, etc.
- You are then scheduled to return to see the same Substance Abuse Counselor generally within a week or two. You will go over the Personal Assessment, Summary, Information and Tips (PASIT) form that is related specifically to your own personal situation. You will receive a copy of this form and then be referred to an appropriate class on or off campus. Classes on campus must be completed within a month. Time frames for completion of off-campus referrals will vary, but need to be completed within the time frame given.
- If referred off campus, a "Notice of Completion" will be sent to your referral source, informing him or her of this referral and the time frame given for completion.
- If referred to an on campus class, a "Notice of Completion" will be sent to your referral source once you have completed the class.
- Please note that if you fail to go to a scheduled appointment or miss a class that you are scheduled to attend, your referral source will be sent a "Notice of Non-Compliance" and you will have 30 days to reinstate the FR process for $25. If you fail to reinstate that process within 30 days, you must get a new referral and pay the $50 origination fee again.
