For your convenience,
we have created the Online Birth Control Education
class that is informative and will help you decide
which birth control method is best for you. This
class will teach you how to properly use your
selected birth control method. Please read the
instructions on this page and the online class
content thoroughly. Review the steps listed below
that explain how to get your birth control at Fronske.
Program Description
This class will take
you about 1 hour to complete. The class consists of
an introduction to contraceptives and eleven short
lessons covering the topics of what to expect at
your physical exam, hormonal contraceptive methods,
non-hormonal methods, IUDs, and fertility and
sterilization methods.
Steps to Get
Your Contraceptive
There are six steps to complete before getting
your birth control at Fronske.
- Step 1:
Decide which birth control is best for you
Please answer
the following question:
Do you
already know which birth control you want to
use?
If you
answered No or Not Sure above,
go to Step 2:
Choosing a Birth Control Method.
If you
answered Yes above, go to Step 3:
The Online
Birth Control Education Class.
- Step 2: Go to
Choosing a Birth Control Method
To help
you choose the right birth control, please
complete the Association of Reproductive Health
Professionals'
Choosing a Birth Control Method guide. This
guide will take about 10 minutes to complete.
- Step 3:
Attend the Online Birth Control Class
Go to the
Online Birth Control
Education Class and read the Introduction
and those sections that apply to your
contraceptive education needs. It is
important that you fully understand how the
contraceptive of your choice is properly used,
side-effects, and contraindications for use. After
finishing your self-study,
go to the next step.
- Step 4: Read
and Sign the Informed Consent Form
After completing
the Online Birth Control Education Class, click on the
Informed Consent form to open it up. This
document describes the benefits and risks
associated with hormonal birth control methods.
Do the following:
- Read the
consent form thoroughly
- Print two
copies of the consent form
- Sign and
date both copies of the consent form
- Keep one
copy for your records
- Bring the
other copy of the consent form to the health
center on the day of your appointment, and
- Give the
consent form to the Patient Services staff
to be included in your medical records
- Step 5: Make
Your Appointment
Make an appointment
for your visit with Fronske's health care
provider for birth
control. Call the appointment line at
928-523-8995. Tell the Patient Services staff
that you have completed the Online Birth Control
Class, that you want an appointment because you will be requesting
birth control. Also, if you need an annual exam,
please inform the Patient Services staff when
you make your appointment for the birth control.

Step 6: Go to the Fronske Health Center
On the day of
your appointment, bring your signed Informed
Consent form. Write down any questions you want
answered concerning your appointment, exam, or
contraceptive. Arrive at least 15 minutes before
the exam to sign in at the front desk in the
Fronske lobby and complete a health history
questionnaire. A Patient Services staff person
will direct you to the appointment location
within the health center. If you need to change
or cancel your appointment, please contact
Patient Services 24 hours before your
appointment time to avoid being charged a late
fee.
