Administrative Add/Drop Request form

You can find and submit the form online by logging into the Central Authentication System (CAS).

Administrative requests must be received within the first 21 days of class for 16 week sessions.  After the 21 days, it is the students’ responsibility to either add or drop their classes.  Shorter session classes have pro-rated deadline, please see the chart below:

Forms and Tools

Request for Special Class Change Form
Used to create dynamically dated classes, change min/max units for variable unit classes, or change grading basis on those classes approved as Both (Letter or Pass/Fail).
**Important Procedures**
  • Request prior to the start of early enrollment for a term.
  • If a unit and/or grading basis change and students are enrolled, it is the responsibility of the department to proactively contact the impacted students, work to solve any resulting student issues, as well as examine/resolve contributing factors of the delayed request.

Used by Flagstaff Academic Departments each time a new topic is created for a course. All terms and instruction modes included. Topics will only be approved for university course lines or officially approved topics courses (e.g. Course Title: Topics in World Music).

Used by Flagstaff Academic Departments to request emplid's for new class instructors who are not NAU employees (not paid by NAU).
You must also:
  1. Request Affiliate status for these instructors through the ITS Solution Center.
  2. Complete the FERPA tutorial in addition to submitting this form.
