Dependent affidavit form for the EARP tuition waiver
You must submit a dependent affidavit if:
- The EARP will be used for a dependent child with a
different last name than the employee.
- The dependent applying for the EARP will be age 24 at any
time during the calendar year in which the EARP is being applied.
- The dependent applying for the EARP is not listed as a
beneficiary on the employee's university benefits (medical, dental, etc.).
- The EARP will be used for a dependent child of a NAU
affiliate.
